Client Rights

 Individuals with developmental disabilities have the same rights as all other citizens of the United States. All individuals/families receiving services from Syringa Family Partnership, LLC. will be informed of their rights in writing as well as verbally upon admission to the program and Client Rights will be posted at all times in all clinic locations. Employees of Syringa Family Partnership, LLC. will participate in training upon hire and throughout their employment to uphold these rights which are detailed below:

 Humane care and treatment

The participant and family will be provided a path to services that is developed with each individual and parent/guardian to actively promote the participation, personal choice and preferences of the participant.

 Not to be put in isolation

This includes not forcing the participant to take a “take a break” unless they are harming themself, or another person.

 Be free from mechanical restraints unless necessary for the safety of that person or for the safety of others

The participant will not be physically restrained (held down or kept from moving) unless they are in danger of harming themself or others.

 Be free from mental and physical abuse

Abuse, in any form, is not tolerated at Syringa Family Partnership, LLC.

 Voice grievances and recommend changes in policies and/or services being offered

If you feel that any of the participant’s or family’s rights have been violated, or you have complaints regarding their program, please follow our Grievance/Complaint Procedure outlined in the Family Handbook. If the participant or family need assistance executing this procedure, help from any Syringa Family Partnership, LLC. staff or an outside agency may be requested at any time.

 Practice their own religion

Syringa Family Partnership, LLC. will not interfere with the participants or the family’s right to practice their religion. Services will be adjusted around religious activities if needed.

 Wear their own clothing and retain and use personal possessions

The participant will not be forced to wear anything they do not want to wear and they may use their own personal possessions. The participant’s right to purchase goods with their own resources, per parent/guardian approval, will also not be restricted.

Be informed of their medical and Habilitative condition, of services available at the agency and the charges for the services

The participant or parent/guardian can ask for more information about their diagnosis at any time to gain a better understanding of their needs and/or abilities. The participant and/or parent/guardian may also request a list of the current reimbursement rates and charges for the services that we have provided them.

 Reasonable access to all records concerning your child

The participant and/or parent/guardian may request access to their records at any time. The request will be granted in a manner as timely as possible.

 Refuse services

Our services are available on a voluntary basis and the participant/family should not be receiving any services that they do not wish to. You may decline any services that are offered or available, and you may discontinue services being provided at any time.

 Exercise all civil rights, unless limited by prior court order

Your right to exercise all other rights guaranteed to you under the Constitution of the United States will not be interfered with, unless limited by a prior court order. Syringa Family Partnership, LLC. may be able to provide you with assistance and/or guidance if needed to exercise your civil rights.

 Privacy and confidentiality

We will not release records or information to anyone other than you without your signed consent. The staff at Syringa Family Partnership, LLC. is trained to protect your privacy, confidentiality and follow HIPPA guidelines.

 Receive courteous treatment

Syringa Family Partnership, LLC. prides itself on hiring individuals that will treat the participant and family with respect and dignity at all times.

 Receive a response from the agency to any request made within a reasonable time frame

Any request you make will be responded to in a timely manner.

 Receive services that enhance the participant’s social image and personal competencies and, whenever possible, promote inclusion in the community

We strive to provide as many services as possible in your local community to help promote inclusion and enhance independence. Because the participant and family are involved in the plan development, they should be working on those goals that you decided were important.

 Refuse to perform services for the agency

If the participant is hired to perform services for the agency, it will have been with your consent and the wage paid shall be consistent with state and federal law.

 Review the results of the most recent survey conducted by the Department and the accompanying plan of correction.

We will be surveyed from time to time by the Department of Health and Welfare to ensure we are following all of the rules to be a licensed Developmental Disabilities Agency. The participant and/or parent/guardian may review the results of this survey and have them explained by Syringa Family Partnership, LLC. staff member at any time. Occasionally, there may be items that we need to correct as a result of this survey and you may also request to review the plan of correction and have it explained as well.

 All other rights established by law, and the agency’s obligation to report abuse, neglect, exploitation, and injuries.

Your right to exercise all other rights established by law will not be interfered with, however, as a licensed Developmental Disabilities Agency we are required by law to report any instance of abuse, neglect, exploitation and/or injuries to the proper departments and/or authorities.

 Be protected from harm

Syringa Family Partnership, LLC. will take steps to ensure that individuals hired do not have a conviction or prior employment history of abuse, neglect, mistreatment, or exploitation. In addition, all confirmed or suspected incidents of mistreatment, neglect, exploitation, or abuse of any individual will be reported to the Department of Health and Welfare and/or the appropriate authorities.
